How do I place a form inside of an email?

Here’s wat I want done:
I want to email a form to someone.
The recipient fills out the form, and then send it back to me.
I want ll this done using (for example) Yahoo Mail. NOT MICROSOFT MAIL!!!!! I want the recipient to be able to simply fill out the form and send it from the email that they recieved, and send it from their email provider.

Send it as an attachment. Word, or PDF. They can fill it out then resend as an attachment
